Role of the NJR Regional Clinical Coordinator


NJR Regional Clinical Coordinators (RCCs) serve to provide two-way communication link between the hospitals in their regions and the surgeons providing data to the registry and to the NJR Management Team staff working for the registry.  The RCC Committee comprises of representatives from all regions across England, Northern Ireland and Wales, from both NHS and independent sector hospitals and acts as a central way for the NJR to ensure the ongoing clinical relevance of its work, creating a channel for suggestions and feedback from RCC members and their regions, to be considered at strategic and operational level.

RCCs also have a key role supporting the improvement of the quality of data in the registry and in turn, the quality of NJR’s evidence to support surgeons and clinical teams at a local level.

Role of the NJR Clinical Liaison Lead


The NJR Clinical Liaison Lead (CLL) role is performed by a designated surgeon or other healthcare professional within all hospitals performing joint replacement surgery.
 
CLLs provide support to their relevant NJR Regional Clinical Coordinator (RCC) and NJR Compliance Officer (CO) as required, facilitating NJR-related considerations and issues that may arise. The CLL will also help support their hospital’s NJR data quality audits.

NJR Data Quality

NJR Data Quality Audits


High-quality data are the foundation of any joint replacement registry and now with around 4.6 million records our valuable data is a fundamental driver to help inform improved quality of care for patients, so it is imperative that the data provides an accurate picture.

The NJR’s programme of work to improve the quality of our data has been a key focus in recent years, driven through
the NJR’s Data Quality Strategy.

Key to this strategy has been our national programme aimed at assessing data completeness and quality within the registry
and we conduct our audit programme to retrospectively check the quality of data within all hospitals reporting to the registry, on an annual basis.

We encourage hospitals to achieve our high targets in these audits through our Quality Data Provider Scheme – you can find details of all our audits, achievement criteria and deadlines for data submissions here.

NJR Consent
Process

Asking patients for their consent to be on the registry


Details of joint replacement operations are recorded in the registry in order to ensure the safety of patients and help to monitor and improve patient outcomes. We also ask patients for consent for their details to be included in the registry so that should there ever be any problems with any implant or procedure type, they can easily be identified for follow-up through their hospital. 
 
All patients having hip, knee, ankle, elbow and shoulder replacement surgery should be given the opportunity to have their procedure details captured in the registry.
Most patients are very happy to have their data included and to date just under 4 million joint replacement records have been captured in the registry.
 
By capturing this information we are able to:

Help identify patients who have received specific implants that are subsequently performing poorly.
Link patients’ primary and revision procedures via the NHS/national patient number, enabling us to determine how long different types of joint replacement implants last.
Invite patients to take part in feedback activities about their surgery, so that patient outcome and satisfaction can be assessed and monitored.

(The National Joint Registry monitors a number of compliance indicators across all hospitals performing joint replacements.
These include linkability, patient consent and engagement with the NJR data quality audit. Where hospitals are not meeting the required targets, the NJR team works with hospitals to identify issues and provide support via the NJR Hospital Escalation Process.)

NJR Outlier Process – Hospitals


Monitoring hospital (unit) performance is one of our key functions as part of the NJR Accountability and Transparency Model which has been in place since 2017. Our monitoring process identifies hospitals with a revision ratio (an indication of implant failure or surgical performance) that is higher than expected.

We have a process in place to contact these hospitals so that an appropriate review of practice can be taken to identify possible causes and actions. Data analyses in the registry are risk-adjusted in order to make a fair comparison between different hospitals.
