How to access the data that we have on the registry about your operation
As a patient, if you gave consent to have your details added to the registry, you can request to see your record/s at any time. This service is available to all patients to obtain the information that we hold on their hip, knee, ankle, elbow or shoulder replacement operation.
Patients should make this request by completing our Patient Data Request Form and sending the signed form to us at the email or postal address below:
